This Waiver and Release of Liability ("Waiver") is entered into on behalf of Praise Tabernacle SDA Church, its parents, affiliates, officers, directors, employees, volunteers, and partners (collectively "Praise Tabernacle SDA" or "the Church") and applies to all participants and recipients of food distributed or provided through the Church's Food Pantry & Giveaway Program (the "Program").
By participating in the Program, you acknowledge and agree to the following terms and conditions:
Participation in the Program is entirely voluntary. No fees are required to receive the food, produce, or grocery items distributed through the Program. Participants may discontinue their participation at any time.
You understand that consuming food may carry inherent risks, including but not limited to allergic reactions, foodborne illness, and other food-related health concerns. You acknowledge that the Church does not guarantee the safety, quality, or suitability of any food items provided through the Program. You assume full responsibility for any risk associated with your participation in and consumption of items from the Program.
To the fullest extent permitted by law, you hereby release, discharge, and hold harmless Praise Tabernacle SDA Church, its parents, affiliates, officers, directors, employees, volunteers, and partners from any and all claims, demands, damages, losses, costs, and expenses arising out of or related to your participation in the Program and/or your consumption of any food or produce distributed or received through the Program.
This release applies to all claims and liabilities, including but not limited to personal injury, illness, allergic reactions, property damage, and any other loss or injury that may occur during your participation in the Program or your consumption of items distributed through it.
You acknowledge that you are solely responsible for your own health and safety. You are advised to use your own discretion regarding whether to consume any food products provided through the Program. You agree to consult with a qualified healthcare professional if you have any concerns about food tolerances, allergies, or other health conditions before consuming any items from the Program.
You agree to indemnify and hold harmless Praise Tabernacle SDA Church and its affiliates from and against any and all claims, damages, losses, liabilities, or expenses that may arise from your participation in the Program or your consumption of any food or grocery products provided through it.
The Church makes no representations or warranties, express or implied, regarding the safety, suitability, or quality of any food item provided through the Program. The source, ingredients, and preparation methods of items distributed are not warranted by Praise Tabernacle SDA Church.
I have read this Waiver in its entirety, fully understand its terms, and sign of my own free will. I understand that by participating in the Program — including but not limited to activities such as lifting, pulling, carrying, transporting, and distributing products within or outside the facility, as well as consuming any products distributed — I am voluntarily waiving legal rights, including the right to bring any claim or legal action against Praise Tabernacle SDA Church and its affiliates for any liability arising from such participation.
